You're in great hands!

All new and existing patients will feel welcome, safe, and respected. During your initial 90 minute consultation, we will review your past medical history, cultural and food preferences, usual diet intake, physical activity, and specific food and nutrition issues. After this we will review some achievable, realistic, and practical food and nutrition goals, to be set by you! You will leave with a good understanding of the direction your headed in, and will receive your nutrition assessment and any specific nutrition information and plan within *24 hrs. Future follow up appointments will be 30 - 60 minutes in duration.*Will notify in advance, if follow up plan later than 24 hrs.

For those that cannot arrange an in-person appointment, a web-based, HIPPA approved, tele-health appointment may be scheduled. More information available upon request.

For an efficient visit, you will be asked to provide health and nutrition history information prior to your appointment.

Or upon request, a “superbill” can be provided for you to submit to your insurance provider for your reimbursement.

Nutrition counseling and/or medical nutrition therapy is eligible for reimbursement through your employer sponsored flexible spending (FSA) or health savings account (HSA).

To finalize the booking of your appointment or service, you will need to return a signed contract and insurance information form; if paying by cash or card (HSA/FSA) you will need to return a signed contract/invoice with pre-payment.
